Repeat-infringer policy

DRAFT - pending attorney review. This is Helmsay's policy for repeat copyright infringers under 17 U.S.C. 512.

Helmsay removes material when a notice is upheld. An account with two upheld removals is a repeat infringer. That account cannot create another gallery post. Helmsay may also close the account. The count uses upheld reports, not rejected ones.
